Dissolving Filler: When It's the Right Call and What to Expect

Dissolving filler carries a reputation it doesn't really deserve. People tend to hear it as an admission that something went wrong — theirs, or their injector's. In practice it's simply one of the tools we have, and it's the reason hyaluronic-acid filler is such a forgiving category to begin with. If a result isn't serving your face, it doesn't have to be permanent. Here's how correction actually works, and how we decide together whether it's the right move.

What hyaluronidase actually does

Hyaluronidase is an enzyme that breaks down hyaluronic acid. Your body already makes its own version of it — it's part of how filler gradually fades on its own over months. Injected directly into a treated area, it speeds that process up dramatically, breaking the gel down so your tissue can clear it.

That also means it only works on hyaluronic-acid fillers: the Restylane, Juvéderm, RHA and Revanesse families, among others. Biostimulators and permanent or semi-permanent products can't be dissolved this way, which is one of the quieter reasons we work almost entirely in HA. Reversibility is a feature, not a fallback.

When dissolving is genuinely the right call

Sometimes correction is the obvious answer, and sometimes it's a preference rather than a necessity. These are the situations where we most often recommend it:

• Migration — product that has drifted beyond where it was placed, most commonly above the lip border.
• Overfilling, where an area reads heavy or distorts the proportions of the surrounding face.
• Lumps, nodules or firm areas that haven't settled after several weeks and gentle massage.
• Asymmetry that placement alone can't correct without adding still more product.
• Old filler layered over the years that's quietly changed the shape of a feature.
• A clean slate before rebuilding — occasionally the fastest route to a natural result is starting over.

There is also a genuinely urgent use: if filler compromises blood flow to an area, hyaluronidase is the emergency treatment, and it's used immediately. That situation is rare, but every injector should be equipped and trained for it — a reasonable thing to ask about before anyone treats you.

When we'll suggest waiting instead

Not every complaint needs an enzyme. Swelling in the first two weeks can look convincingly like overfill and then resolve completely on its own. A small firm spot at three days is often just the tissue settling. And if what's bothering you is really a proportion issue, supporting a neighbouring area may balance the face far more gracefully than removing what's already there. We'd rather reassess you at two weeks than dissolve something you'd have loved once the swelling passed.

Dissolving is not a precision instrument. Hyaluronidase acts on the hyaluronic acid it reaches, which can include some of your skin's own — so we dose conservatively, treat the specific area, and never use more than the situation calls for.

What the appointment is like

It's shorter than you'd expect. After we assess and map the area, the enzyme is injected in small amounts directly into the filler, sometimes with numbing beforehand. Most of the breakdown happens within 24 to 48 hours, though the area can look swollen, uneven or oddly flat during that window — that's normal, and it isn't your final result. We'll usually see you again at about two weeks to assess properly, and a second session is sometimes needed for stubborn or long-standing product.

Refilling afterward

If you plan to treat the area again, we generally wait at least two weeks — often longer — before placing new product. The tissue needs time to calm down and settle back into its own shape, and rebuilding on a swollen, freshly dissolved area is how people end up back where they started. When we do refill, we go slowly and conservatively, which is the same philosophy behind everything we do: undetectable injectables, built in stages rather than all at once.
