Bruising & Swelling After Injectables: What's Normal and What Helps

A little bruising or puffiness after Botox or filler isn't a sign something went wrong — it's usually just your body responding to a needle passing through skin and tissue. But because so much online chatter conflates "normal healing" with "botched," we get this question constantly: is this swelling okay, or should I be worried? Here's the honest, medically grounded answer.

Why bruising and swelling happen at all

Every injection, no matter how precise, creates a tiny disruption in tissue. Filler also physically displaces space under the skin, and your body responds with fluid and inflammation as part of normal healing. Some areas bruise more easily than others simply because they're more vascular — lips, under-eyes, and the area between the brows are the usual suspects. None of this reflects your injector's skill; it reflects your anatomy that day.

What's considered normal

  • Mild to moderate swelling in the first 24–72 hours, often worse the morning after
  • Pinpoint bruising at injection sites, ranging from faint pink to deep purple
  • Tenderness or firmness in treated areas for several days to a couple weeks
  • Asymmetry in swelling or bruising between the two sides of your face
  • Lip filler and under-eye filler tend to swell the most dramatically of any areas we treat

Bruises typically move through a color cycle — purple to green to yellow — before fading entirely, usually within 7 to 14 days. Swelling resolves faster, often settling within a few days, though filler can take up to two full weeks to show its final, settled shape.

What helps in the first 48 hours

  • Ice in short intervals (10 minutes on, 10 minutes off) during the first day
  • Keeping your head elevated, even while sleeping, for the first couple of nights
  • Avoiding alcohol, intense exercise, and hot yoga or saunas for at least 24 hours
  • Skipping blood-thinning supplements like fish oil and high-dose vitamin E until healed
  • Arnica tablets or topical arnica gel, started the day before treatment if bruising is a concern

We walk every client through an aftercare plan specific to what they had done, because lip filler aftercare looks a little different than neurotoxin aftercare or under-eye filler recovery. If you want the full breakdown before your visit, our aftercare guide covers it treatment by treatment.

When swelling or bruising is NOT normal

This is the part worth reading closely. Most swelling is cosmetic and temporary. But certain signs warrant a call to your injector right away, not a wait-and-see approach.

  • Swelling that worsens significantly after day 3 instead of improving
  • Skin that turns dusky, mottled, or notably pale near the injection site
  • Increasing pain rather than gradual improvement
  • Fever, spreading redness, or warmth that suggests infection

These red flags are rare, but they're the reason every Wellness House client leaves with direct access to medical staff-led follow-up, not just a printed aftercare sheet. If something feels off, we want to hear from you.

Can you reduce your odds of bruising before you even book?

Somewhat, yes. Avoiding alcohol, NSAIDs like ibuprofen, and fish oil for a few days before your appointment can meaningfully lower your risk. So can scheduling treatments away from big events — we generally suggest at least two weeks of buffer before a wedding, reunion, or photoshoot, precisely because bruising and swelling timelines vary person to person.

During your consultation, we'll also ask about your medications, supplements, and bruising history, because that's part of what good faith exams and thoughtful injecting are for — minimizing surprises, not just treating what's in front of us.

Bruising and swelling are part of the deal with injectables, but how they're managed shouldn't be left to guesswork. If you're preparing for your first appointment or your fifth, we're happy to talk through what to expect for your specific treatment plan.
