Every week, someone asks us some version of the same question: shouldn't I get Botox from a dermatologist? Isn't a plastic surgeon safer for filler? It's a fair question — and the honest answer isn't about the letters after someone's name. It's about who is actually holding the needle, how often they do this specific work, and how they think about your face.

The credential myth

Here's what surprises most people: dermatologists and plastic surgeons are trained primarily in their own specialties — skin disease, or surgical reconstruction and body procedures. Injectables are often a small slice of their practice, sometimes delegated entirely to a nurse injector on staff. Meanwhile, a med spa built around injectables may have an owner-injector who does nothing else, all day, every day.

So the real question isn't 'What's their degree?' It's 'How much of their actual practice is this exact treatment?' A board certification tells you someone passed an exam years ago. It doesn't tell you how many syringes of filler they've placed this month, or whether they specialize in undetectable injectables versus reconstructive work.

What each setting is actually built for

  • Plastic surgeons: excellent for surgical procedures — facelifts, rhinoplasty, body contouring. Injectables are often a side offering, sometimes handled by an associate injector rather than the surgeon.
  • Dermatologists: excellent for skin health, moles, rashes, acne, skin cancer screening. Injectables may be one small part of a very medical, very busy practice.
  • Dedicated injectable studios / med spas: built entirely around neurotoxin and filler work. The injector's continuing education, artistry, and daily reps are focused on facial anatomy and injectable technique specifically.

The questions that actually matter

Instead of asking about degrees, ask about the injector directly. Who will actually be doing my treatment? How long have they been injecting, and is this their primary focus? Can I see real client results? What happens if something needs to be adjusted or dissolved later? A confident, experienced injector will answer these plainly — no hedging, no upsell pressure.

In Texas, injectables are performed under a supervising physician's protocols regardless of setting. What varies is who's actually behind the needle and how much of their practice is devoted to this work — that's the detail worth asking about.
